Fauquier Health honors Planetree award recipients

Dr. Jeffrey Abrams, informaticist, earned the Spirit of Planetree Clinician Champion award.
Fauquier Health recently honored three “Spirit of Planetree” recipients, healthcare professionals whom peers nominated for “going above and beyond to support or deliver extraordinary patient and resident-centered care.”

The Warrenton hospital years ago adopted the Planetree model that focuses intently on patient care and comfort. To achieve Planetree designation, it had to meet more than 60 performance criteria in 11 core areas.

Fauquier Health’s honorees:

• Spirit of Planetree Caregiver Award

Richard Shrout, an oncology nurse practitioner, received the Caregiver Award for Fauquier Hospital.

“I have never witnessed someone like Richard,” his nomination reads. “From a bedside nurse, to managing clinical trials, to his current role as an oncology nurse navigator, Richard has never lost sight of the patient as the center of everything he does.”

• Spirit of Planetree Clinician Champion Award

Dr. Jeffrey Abrams, physician informaticist, earned the hospital’s Clinician Champion Award.

The award recognizes physicians or advanced practice clinicians who champion the Planetree model of patient-centered care and whose actions make him or her a role model for other medical staff members.

“Dr. Abrams’ focus on patient care and his support crosses disciplines and hierarchies,” his nomination reads. “To ensure we improve our care - he makes sure that clinicians, nurses, physicians and administrators have the data they need to understand the problem first. He then looks for solutions where technology helps us provide higher touch/higher quality care.”

• Spirit of Planetree Animal Therapy Award

Sassie, an Aussie/border collie — along with her handler Kathryn Gilmand and pet rep Tiffany Merkley — received the Spirit of Planetree Pet Therapy Award.

“What a beautiful and kind pet therapy team!” their nomination reads. “Sassie is so sweet and gentle. I feel so blessed that we have animals that instinctively know how to be gentle and that also bring so much love to patients and employees.”

• 2016 Spirit of Planetree Excellence in Teamwork Award

Planetree also chooses special programs and initiatives that benefit patients. The Excellence in Teamwork Award recognizes exemplary team approaches to creating and implementing programs that enhance the patient and family experience.

The Culture of Safety team at Fauquier Heath earned the award for implementing tools and processes that create and support an environment in which individuals and teams are hyper-vigilant about safety. The team put in place new processes and systems to assure patient safety.

Members of the team came from many different departments: Laura Welsh, Claudia Dornin, Fran Spearman, Margaret Rowe, Karla Kenefake-Hymens, Cindy Hobbs-Witmer, Colleen Beres, Christie Shaw, Ina Bowman, Eric Maroyka, Melissa Finley, Brenda Hartshorn and Alissa Nobblitt.

The Villa, Fauquier Health’s assisted living facility, recognized Carol Fox as the Spirit of Planetree Caregiver of the Year and Casey, a furry favorite with residents, as the Pet Therapy recipient.

The Fauquier Health Rehabilitation and Nursing Center recognized Brenda Payne as the Spirit of Planetree Caregiver of the Year and Dr. Ami Behiri as the Clinician Champion. Alvin, a cat from Paws for Seniors was the Pet Therapy Award recipient.
